I have not raised a dispute with the credit card agency. But I want to be very clear regarding my request. Your website appears to be all encompassing by taking my personal information, which is very sensitive and private, and processing my request without my having to incur further costs and / or processing and fees. However, when being delayed several days by being provided with the false form and having to text and email with different individuals responding to our requests for assistance in getting this processed properly and accurately, you should understand that we loose your credibility. And then be required to complete the form ourselves, after giving you our personal sensitive information required, and having to pay additional state fees along with postage fees, I loose all hope when dealing with your company. <br />

I have no idea how long your company has been in business, but in dealing with my request it appears your company has very little knowledge of how to adapt to individual processes. <br />

For my request, you could have just simply sent me a link to the form, indicating that you are not allowed by the State to submit our request. This would have cost me considerable less time, money and aggravation. <br />
<br />
As for you indicating that you have applied my application to the State, I called Vital Statistics and asked if any orders existed on our behalf from your company and the lady placed me on hold and was researching the system and later returned indicating there was no record of any orders. <br />
<br />
Also, on your cancellation form, there is no field available for customers to furnish the reason for cancellation. I guess you don‘t want that information to accompany the cancellation request.<br />

We have found the form on the internet after telephoning Texas Vital Statistics, printed and filled out the form and have taken it to our local post office and have paid 75€ to send it from Germany to Texas. I requested to have the form sent to my daughter who will forward the form to us. We will forward the required document to the Frankfurt U.S. Embassy for final processing of our subsequent efforts.<br />
